8-2-3: PERMIT REQUIREMENTS:
   A.   Driveway/Culvert Permit: No person shall begin construction or repair of driveway culverts or underdrain systems without first submitting a completed application for a driveway or culvert permit on a form approved by the village and without having secured said permit from and paying the applicable fee to the village hall. The village, however, shall not issue any permit under this chapter for any private accessway or for the breaking or removing of any curb where the proposed opening would create any undue hazard to pedestrians or create a traffic hazard.
   B.   Excavation/Utility/Curb Cut Permit: No person shall break or cut away any portion of any curb and gutter, street pavement, or sidewalk pavement now or hereafter laid or established on any street, alley or public way without first submitting a completed application for an excavation/utility/curb cut permit on a form approved by the village and without having secured an excavation/utility/curb cut permit from the village hall.    C.   Notice Of Construction; Plans And Drawings: Any person intending to begin such construction or removal shall satisfy any requirements for notice of construction and requirements for submission of plans or drawings so enumerated on the permit instructions. Also, before a permit will be issued for a curb cut onto a state right of way, a copy of an approved permit from the state of Illinois department of transportation shall be submitted to the streets and public utilities committee.    D.   Inspections:
      1.   Before Permit Issuance: Before issuance of the permit, the village superintendent of public works may inspect the site of the proposed construction and may inform the applicant in writing of any changes to the plans or drawings necessary to ensure compliance with this chapter. Also, any person installing an accessway through a curb break shall notify the superintendent of public works not less than forty eight (48) hours prior to the commencement of construction.
      2.   After Construction: After completion of the construction, the applicant shall so notify the village superintendent of public works who shall inspect the completed construction to ensure that it has been built in compliance with this chapter and with any requirements specified upon the issuance of the permit.
   E.   Fees: The driveway culvert permit fee shall be fifty dollars ($50.00). (Ord. 2009-107, 9-8-2009)
