A. Applications for all licenses and permits required by this code shall be made in writing to the village clerk unless otherwise specifically provided by law.
B. Each application shall, at minimum, state the following:
1. Name of the applicant;
2. The permit or license desired;
3. The location to be used, if any;
4. The date and time to which the permit or license applies;
5. The fee to be paid; and
6. Such additional information as may be required by the issuing official. (Ord. 2010-05, 3-15-2010)
