§ 150.03  PERMIT.
   (A)   No mobile home shall be placed within the limits of the village without first securing a permit from the designated village inspectors. An application form for such permit shall be obtained from the Village Clerk or inspectors. The completed form shall be filed with the Village Clerk and shall contain the following:
      (1)   Description of the mobile home, including year, model, and identification number;
      (2)   Area of the lot in square feet;
      (3)   Size of mobile home, length and width;
      (4)   Lot number, block number, and name of the subdivision on which the mobile home will be placed;
      (5)   Description of the type of underpinning to be used;
      (6)   Type of sanitary sewer connection;
      (7)   Type of wiring and electrical system;
      (8)   Type of material to be used for skirting;
      (9)   Date of fire extinguisher last inspection;
      (10)   A scale drawing showing the location of the mobile home on the lot and the setback measurements;
      (11)   A photograph of the mobile home; and
      (12)   A list of all defects in the exterior covering including, but not limited to, holes, dings, rust, scratches, flaking paint, missing covering, and broken windows and screens.
   (B)   A fee in an amount as set by the Board shall be paid to the village inspectors at the time the application is filed.
   (C)   If the village inspectors approve the application, then they shall issue the applicant a permit on a form supplied by the Village Clerk and a copy of which shall be filed with the Village Clerk.
   (D)   All applicants shall be given a copy of this chapter with the application form.
