No person shall do any of the following:
(A) Offer tattooing, body piercing, or other body modification services, without a valid permit issued by the Health Officer;
(B) Perform a tattooing, body piercing, or other body modification procedure in a manner that does not meet the requirements of this chapter, 410 I.A.C. 1-5, and I.C. 35-45-21-4; and/or
(C) Tattoo removal procedures, unless said procedures, or person, is approved by the Food and Drug Administration.
