The following incidents should, as a matter of routine, be investigated and a written report prepared and delivered to the City Manager immediately:
(A) A firearm is discharged by a police officer in the line of duty.
(B) An accident occurs involving a police officer, either in pursuit or directly.
(C) A police officer or other party is injured during the performance of an officer's duties.
