Any user who violates any of the following conditions of this subchapter, or any applicable state and federal regulation, is subject to having its permit terminated:
(A) Failure to report accurately the wastewater constituents and characteristics of its discharge;
(B) Failure to report significant changes in operations, or wastewater constituents and characteristics;
(C) Refusal of reasonable access to all portions of the user’s premises and records for the purpose of inspection or monitoring; or
(D) Violation of any conditions of the permit or of this chapter and Chapter 51.
