(A) Applicant shall pay a fee of $50 for the issuance of an initial one-year permit and an annual renewal fee thereafter of $25.
(B) A verified annual report shall be submitted certifying that there have been no changes in operational procedures, or if there have been such changes, furnishing the information thereof in such detail as may be required by the town. Failure to submit such report shall constitute cause for the suspension or revocation of the industrial waste discharge permit. In the event a permit is cancelled for any cause under the provisions hereof, a fee of $150 shall be charged for a subsequent initial permit issued to such applicant by the General Manager.
