(A) Written application shall be made in writing at the office of the City Clerk.
(B) The application shall state the name and address of the owner of the dog, the name, breed, age, sex, and color of the dog, and be accompanied by a certificate of vaccination from a qualified source certifying that the dog has been vaccinated against rabies, or has received a booster shot therefor, within the 24-month period immediately preceding the application.
