9-217: APPLICANT TO FURNISH REPORT OF PHYSICAL EXAMINATION:

Within five (5) days from the date of filing an application for a license under this chapter, the applicant shall submit a report of a physical examination (not over 1 year old) from a duly licensed physician. (Prior Code, Sec. 5-27)
