9-216: FILING AND CONTENTS OF APPLICATION:
   A.   Each applicant for a license under this chapter shall make written application on forms available at the police department. Such application shall contain the following information:
      1.   Private security officer for guard service:
         a.   Company, if any, employing the applicant;
         b.   A standard personal history statement completed on a form provided by the chief of police; or
         c.   Other and additional information deemed necessary by the chief of police;
      2.   Private security company for guard service:
         a.   Names of the officers, principals or resident managers of the company;
         b.   A standard personal history statement for such persons, completed on a form provided by the chief of police; or
         c.   Other and additional information deemed necessary by the chief of police;
      3.   Private security officer for patrol service:
         a.   The specific place or area where services would be rendered;
         b.   Hours of the day when services would be rendered if the applicant is self-employed;
         c.   The company, if any, employing the applicant;
         d.   A standard personal history statement completed on a form provided by the chief of police; or
         e.   Other and additional information deemed necessary by the chief of police;
      4.   Private security company for patrol service:
         a.   The names of the officers, principals or resident manager of the company;
         b.   A standard personal history statement for such persons completed on a form provided by the chief of police;
         c.   The specific area or properties where the service would be furnished, and the hours of the day that service would be furnished;
         d.   The names of the officers who are to service the area; or
         e.   Other and additional information deemed necessary by the chief of police. (Prior Code, Sec. 5-26)
