§ 93.21 APPLICATION FOR PERMIT.

   Applications for permits shall be made to the Village Treasurer and shall state the following:

   (A)   Location of the intended excavation or tunnel;

   (B)   The size thereof;

   (C)   The purpose therefor;

   (D)   The person doing the actual excavation;

   (E)   The person for whom the work is being done;

   (F)   The date when the work is to be started; and

   (G)   The estimated date for completion thereof.
