Upon proof by report from the Inspector of the Department of Health that the premises on which a poultry slaughterhouse permit has been issued by the Department of Health has become vacant or has not been used or is discontinued for use as a slaughterhouse for the period of six months or more, or where the permittee has died, or where the premises has been destroyed by fire, a new permit must be obtained and new plans and specifications filed, passed upon and approved by the Department of Health, as herein provided.
