§ 112.005 APPLICATION FORMS.
   Application for a license shall be made on forms provided by the city and shall contain the following:
   (A)   The name, age and address of the applicant; and
   (B)   The location where the business of renting or selling of motorcycles will be conducted and the number of motorcycles that will be maintained on the premises.
