§ 152.24 DATA FOR FINAL PLAT APPROVAL.
   Final plat shall be drawn in ink on mylar that shall be 18 inches wide by 24 inches long and shall be at a scale of 100 feet to one inch. The Plan Commission may require a scale of 50 feet to one inch in appropriate cases. Where necessary, the plat may be on several sheets showing the entire submitted for approval progressively in contiguous sections satisfactory to the Plan Commission. The final plat shall show the following:
   (A)   Name of subdivision.
   (B)   Location by township, section, town and range or by other legal description.
   (C)   Scale one inch to 100 feet, unless otherwise indicated.
   (D)   Date, north point and basis of bearings.
   (E)   Boundary of plat, based on a closed traverse (implying all side and interior angles being measured by proper and accepted field methods) with a maximum error on boundary dimensions and closure of 1:5000. Closure calculations must be included and the Indiana State Plan Coordinate System shall be utilized.
   (F)   Exact location, width and name of all streets within and adjoining the plat, and the exact location and widths, of all cross-walkways. Streets that are obviously in alignment with others already existing and named shall bear the names of the existing streets.
   (G)   True angles and distances to the nearest established street lines or official monuments which shall be accurately described on the plat.
   (H)   Municipal, township, county or section lines accurately tied to the lines of the subdivision by distances, angles, and bearings.
   (I)   Radii, central angles, point of curvature tangent bearings, and lengths of all arcs of street lines.
   (J)   Location, dimensions and purposes for all easements, ditch, and drain locations and the location of designated flood hazard areas.
   (K)   All block and lot numbers and lines, with accurate dimensions in feet and hundredths with bearings given for all lines.
   (L)   Location and description of permanent monuments or bench marks.
   (M) Accurate outlines and legal descriptions of any areas to be dedicated or reserved for public use, with the purposes indicated thereon, and of any area to be reserved by deed covenant for common uses of all property owners.
   (N)   Building setback lines accurately shown by dimension.
   (O)   Protective covenants which meet with the approval of the Plan Commission shall be lettered on the final plat or attached thereto.
   (P)   A summary on its face of all restrictions applicable to any part of the subdivisions relating to building restrictions, use restriction building lines or otherwise.
