2. Describe the building to be moved, including its size.
3. State the reason for moving the building.
4. State the use, purpose and occupancy for which the building is to be used after it is moved.
5. Give the present location of the building.
6. Give the location to which the building will be moved.
7. If the new location is to be in the Town, attach a plot plan showing the proposed location of the building upon the property: (See attached)
8. List all of the streets on, over, or through which it is desired to move said building and describe the route.
9. Does the zoning permit the building in the new location?

10. State the name, address and phone number of the contractor moving the building.

I/we acknowledge receipt of Ordinance No. 2000-03, regulating the moving of buildings, and agree to be bound by the terms of that Ordinance if the Town of Hamilton grants a moving permit.

INVESTIGATION CHECK LIST

2. Verify zoning of the new location and whether the structure is permitted by zoning in the new location.
3. State the required cash deposit to cover the costs described in division (F) of the Ordinance.
4. Obtain Certificate of Liability Insurance as required by the town.
5. Obtain completion bond required by division (F) of the Ordinance.
6. Has approval been obtained from the following utilities:
(a) Town Water Department;
(b) Regional Sewer District;
(c) Electric Company;
(d) Gas Company;
(e) Telephone Company;
(f) Cable TV Company.
