(A) Application for permit. All applications for permit under the provisions of this subchapter shall be in the name of the ultimate carrier which is to be the final owner of the system giving its name, address, and phone number and (if an LLC) the names, addresses, and telephone numbers of the managing partner and the owners. The application shall be signed by the ultimate owner as well as the contractor that is to do the construction designated in the application. Such application shall not be assignable, and no person shall allow his or her name to be used to obtain a permit for any other person. Applications shall be made on forms provided for this purpose, a copy of such application being attached hereto
(B) Permit form. A permit form shall be provided by the City Clerk in the form attached to the ordinance codified herein.
