Each mobile food vendor shall apply for a permit issued by the City Clerk, and the City Clerk shall issue a permit upon receiving from the mobile vendor applicant copies of:
(A) The vendor’s County Health Department permit;
(B) The vendor’s State Sales Tax ID number;
(C) Proof of insurance;
(D) A copy of the owner’s driver’s license; and
(E) Payment of a fee of $5 for a one-year permit, which permit must always be prominently displayed while vending.
