Applications for permits shall be made on forms provided by the council and shall contain information as may be deemed necessary by the council, or its designated agent, and the form shall contain a provision that all work contemplated shall be performed in compliance with all applicable laws and ordinances of the town and any regulations relating thereto, and the form shall contain a representation by the applicant that he shall indemnify and save harmless the town from any and all damages to persons or property resulting from, or caused by, any act or omission in the performance of the work to be done.
