Any person so possessing such a device shall make application for a license with the Borough Manager in writing and under oath, setting forth:
(A) His or her present and previous occupations;
(B) The length and place of his or her present and previous residences;
(C) The name and address of the owner of the premises where the devices are to be used;
(D) If the owner of the premises is not the applicant, the length of the term of the lease for the premises;
(E) If the possessor is not the owner of the device, the name and address of the owner of the said device;
(F) The location where the machine or device is to be displayed or operated and the business conducted at that place; and
(G) The type and serial number of the device sought to be licensed.
