725.03   INSURANCE REQUIRED FOR PRIVATE HAULER.
   The contractor shall secure, pay for and maintain until completion of the contract, the following:
   (a)   Public liability and property damage insurance as shall protect him and the City, as named insured, from claims for personal injury or property damage which may arise because of the nature of the work or from operations under this contract.
   (b)   General liability shall be on a per occurrence basis, and in comprehensive form. General liability coverage shall include broad form property damage and personal liability. The contractor shall provide general liability and excess general liability in the following amounts, at a minimum: two million dollars ($2,000,000) primary general liability plus three million dollars ($3,000,000) excess general liability. The contractor will provide a certificate of insurance on the anniversary of this contract to the Director of Public Service. Failure to provide or maintain this certificate may result in immediate termination of this contract.
   (c)   Coverage for any auto, including non-owned and hired autos, with a combined single limit of one million dollars ($1,000,000) per occurrence. In addition, the contractor shall provide uninsured and underinsured motorist coverage in the amount of one million dollars ($1,000,000) per occurrence.
