§ 110.02 APPLICATION.
   Any person desiring a license for any such business or occupation shall file application therefor with the City Clerk/Finance Director upon a form to be provided by the City Clerk/Finance Director for that purpose, which shall set forth the name of the applicant, address and business or occupation for which such license is desired.
