§ 153.1649 NON-CONFORMING USE CERTIFICATION.
   (A)   Number of copies. To establish non-conforming rights, the owner or his or her designated representative must file three copies of the required documents with the Code Enforcement Officer.
   (B)   Format. The applicant shall provide a site plan, including an eight and one-half-inch by 11-inch reduction of the plan on vellum or equivalent material, and narrative material as required. The site plan shall indicate proposed and existing uses as well as the appropriate zoning designation.
   (C)   Contents. As a minimum, the documentation shall include:
      (1)   Evidence of financial investment;
      (2)   Affidavit of ownership;
      (3)   Narrative explanation of the proposed project and its status; and
      (4)   Any development approvals previously approved by the city, including the type of development approval, date of approval, approving agency and scope of approval, including all uses, buildings, structures or other activities authorized by the development approval.
