151.05 INSURANCE.
   (a)   The Employer shall make available to all Employees hospitalization/major medical, vision, life and dental insurance plans.
   (b)   (1)   All Employees enrolled in the health care programs shall contribute fifteen percent (15%) of their applicable monthly premium toward the cost of providing insurance benefits. Such contribution shall be made through regular payroll deduction.
      (2)   All Employees enrolled in the life insurance plan shall contribute twelve percent (12%) of their applicable monthly premium toward the cost of providing insurance benefits. Such contribution shall be made through regular payroll deduction. (Ord. 25-17. Passed 12-6-17.)
   (c)   Whenever gross savings (i.e. lower premium prices) become available to the Village of Greenfield as a result of the Employee committee process, the Employer agrees to share fifty percent (50%) of this savings with the Employees. The Employer agrees to share this savings by dividing it equally between all enrolled Employees and make a one time lump sum payment to each Employee or a one time lump sum contribution payment at the Employee's request into said Employee's HSA account no later than December 31 of the year in which the savings was realized by the Employer. This one time lump sum payment will be in addition to the Employer's HSA contribution of $1,250 for single-coverage and $2,500 non-single coverage.
   (d)   The Village of Greenfield agrees that should an Employee waive all available hospitalization/major medical insurance coverage, he/she shall receive a rebate of one thousand dollars ($1,000) payable to the Employee, the second payday following each insurance calendar year and in a separate check. Should the Employee wish to reinstate their coverage, he/she shall wait until the next open enrollment period, unless a qualifying even would occur.
   (e)   The Village of Greenfield agrees to maintain in force the present hospitalization/ major medical programs for an Employee who encounters a work place injury for a period of ninety (90) calendar days following the exhaustion of the Employee's accumulated sick leave.
   (f)   The City Manager shall create an Employee committee to seek proposals from bona-fide health insurance brokers, review same and recommend health insurance programs to the City Manager who shall then recommend appropriate health insurance programs to the Village Council.
