§ 150.14  PROCEDURE FOR NEW/CHANGE OF ADDRESS.
   The Clerk shall provide a new/change of address form to be completed by any person residing or intending to reside in the unincorporated areas of the county who needs to obtain a new or change of address. The new/change of address form must be sent to the Clerk for processing. In the event an incorporated city or town adopts this chapter by resolution, the Clerk shall provide new/change of address forms to the incorporated city or town, and the city or town shall forward any new or change of address request to the Clerk or direct the requesting party to send the completed form to the Clerk.
