§ 110.09 APPLICATION FOR LICENSE.
   Application for a license shall be made in writing to the Clerk-Treasurer upon the forms as provided by the city and shall include all names by which the applicant has been known or identified, all addresses of residences or places of business maintained by the applicant, a description of any traffic, criminal or regulatory charge, official notice, finding of liability, conviction or other determination made by any governmental or regulatory agency, unit or other entity. If the applicant is a business entity, the information shall be provided for each partner, officer, director, or other principal thereof. Included with the application, the applicant shall provide a driving record and criminal history background report.
