(A) Each permit holder must complete a city quarterly report form and file it with the Director of Public Service. The report form must be submitted by April 15, July 15, October 15 and January 15 for the preceding quarter.
(B) The report will contain the number of units serviced within the city, the weight in tons of refuse collected within the city, the weight in tons of recyclables collected within the city and the weight in tons of yard waste collected within the city, as well as the name of the facilities to which the materials were taken.
(C) Permit holders who do not file their reports pursuant to division (A) of this section shall be fined $20 per day for each day not in compliance.
