Every petition for appeal shall
(A) Be in writing;
(B) Identify by full name and mailing address each person or entity filing the petition for appeal;
(C) Be accompanied by a copy of the Condemnation Notice;
(D) Include a request for a hearing;
(E) Contain a brief statement as to the grounds for the appeal; and
(F) Be filed with the Board of Zoning Adjustments and Appeals within ten (10) days from the date the Condemnation Notice was served on the person(s) or entit(ies) filing the petition for appeal with the Board of Zoning Adjustments and Appeals.
