§ 30.301   PLANS AND SPECIFICATIONS; APPROVAL; CONSTRUCTION
   (A)   Definition. For the purpose of this sections, the following definition shall apply unless the context indicates or clearly requires a different meaning.
   “WATER LINE EXTENSION.” The construction and/or installation of a water line attached to the existing city water system and receiving potable water from the system. Any water service line that extends more than one hundred fifty (150) linear feet or crosses more than one (1) property line shall be termed a water line extension irregardless of meter placement.
   (B)   Plans and specifications.
      (1)   Plans and specifications for all public water lines shall be prepared and submitted to the Utilities Commission by a professional engineer registered in Kentucky, and shall bear his seal. Construction or installation of any new works shall not begin until four (4) copies of the plans and a set of specifications provided by the city, along with any additional design data that may be required for proper review of the plans. These plans once approved by the city, must be submitted to the Division of Water and their approval must be submitted back in writing.
      (2)   A complete package, including plans, specifications, necessary fees, letters, and other information, shall be submitted in such form and content as may be specified by the city and the cabinet, and shall be submitted at least thirty (30) days prior to the date on which action is requested of the cabinet.
      (3)   The front page shall contain the name of the water supply, its ownership by city and county, and the name of the person preparing the plans.
      (4)   The plans shall be drawn to scale and accompanied by proper specifications, so as to permit a comprehensive engineering review.
   (C)   Approval of final plans. If approved, one (1) set of plans and specifications stamped “approved” will be returned to the engineer and one (1) to the city.
   (D)   Construction.
      (1)   A set of approved plans and specifications shall be available at the job site at all times. All work shall be performed according to them.
      (2)   If the cabinet's or the city's representative observe work being performed in a manner which does not conform to the approved plans and specifications, the job may be shut down and no water placed on it.
   (E)   Final approval of facility.
      (1)   Upon completion of construction, the person who presented the plans shall certify, in writing, that the project has been complete in accordance with the approved plans and specifications.
      (2)   After the one (1) year warranty period, the city will incorporate the line into its water distribution system.
   (F)   Expiration of approval. If construction has not begun within one (1)year from the date of approval, the approval shall expire. Extension of approval may be granted upon written request to the Division of Water.
   (G)   Recommended standards for water work. All plans and specifications pursuant to this regulation shall be consistent with “Recommended Standards for Water Work” and “Water Policy Memorandum #84-02”, general guidelines for conducting stream studies for wastewater discharge proposed within five (5) miles upstream of a public water supply source, or for the location of public water supply intake within five (5) miles downstream from wastewater discharge, which are incorporated by reference herein.
   (H)   Standard specifications.
      (1)   All lines installed shall be at least Schedule 21 Class 200 PVC Bell Joint Pipe. The extension of two inch (2”) water lines is hereby forbidden unless the line needed to tie into is two inches (2”). Extensions for subdivisions shall be no less than six inches (6”) so fire hydrants may be installed.
      (2)   All water lines will be no less than thirty-six inches (36”) deep with specific requirements being greater in some circumstances.
      (3)   All backfill material shall be an acceptable fine grain material, with no rock placed within one and one-half feet (1 ½') distance of the pipe.
      (4)   All dead-ends must have a blow-off assembly the same size as the pipe up to a four inch (4”). A six inch (6”) and eight inch (8”) can use a four inch (4”) assembly.
      (5)   All lines must be disinfected per Division of Water regulations under the supervision of the city. A test will be performed after the twenty-four (24) hour period to ensure the 25 ppm residual.
      (6)   Water lines cannot be placed within ten feet (10') of an existing sewer horizontally.
      (7)   Pressure calculation will be performed to ensure a minimum of thirty-five pounds (35 lbs.) will exist at the highest delivery point and the last location in a line and delivered to the city in writing.
      (8)   Upon completion, the applicant shall produce a certification that the line was installed according to plans and specifications for delivery to the Division of Water. This must be signed and sealed by a licensed professional engineer in the State of Kentucky, and certified as built. This certification must be delivered to the city.
      (9)   Air relief valves shall be placed at appropriate locations whenever elevation changes warrant their use.
      (10)   All fittings and valves shall be mechanical joint fittings.
      (11)   The blow-off shall be equipped with a standard valve box.
      (12)   If the city supplies the inspector, it will be at the expense of the installer.
