(A) Significant industrial users proposing to discharge into the POTW, must first obtain a wastewater contribution permit from the city. Any user who violates the following conditions of this article or a wastewater contribution permit or order, or any applicable state or federal law, is subject to permit termination for the following:
(1) Failure to actually report the wastewater constituents and characteristics of its discharge;
(2) Failure to report significant changes in operations or wastewater constituents and characteristics;
(3) Refusal of reasonable access to the user's premises; or
(4) Violation of conditions of the permit.
(B) Noncompliant industrial users will be notified of the proposed termination of their wastewater contribution permit and be offered an opportunity to show cause why the proposed action should not be taken.
