(A) Annually, each establishment in which there is one or more coin-operated amusement device shall obtain a license for the establishment, and shall register all such devices, as provided in this chapter. The license term shall commence on January 1 and expire at the end of December 31.
(B) All persons operating coin-operated amusement devices in the village as of the effective date of this chapter shall have a grace period to and including January 31, 2006 in which to obtain a license for calendar year 2006. Thereafter, annual licenses shall be obtained in December of each year for the upcoming year.
(C) The Village President shall be the licensing authority for the village and shall grant an establishment license upon submission of an application on a form prescribed by the Village President, together with the applicable license fee and registration fees. The form shall include, as a minimum:
(1) The name, address, and telephone number of the establishment;
(2) The name, address, and telephone number of all persons having more than a 5% interest in the establishment;
(3) The name, address, and telephone number of at least one manager of the establishment;
(4) The name, address, and telephone number of the owner of the coin-operated amusement devices in the establishment;
(5) The make, model, and serial number of all coin-operated amusement devices in the establishment as of the time of application; and
(6) A statement that no coin-operated amusement device will be used for gambling.
(D) No license shall be granted to any establishment where any manager or owner of more than a 5% interest of the establishment has been convicted of a gambling offense in the previous five years.
(E) No license shall be granted to any establishment which has had a license revoked within the previous year.
