   The following are FOIA forms:

   (a)   Request for public records form;

   (b)   Notice to extend response time form;

   (c)   Notice of denial form;

   (d)   Detailed cost itemization form;

   (e)   Appeal of denial of records form; and

   (f)   Appeal of excess fee form.
