(a) Applicants for a license under this chapter file with the City Clerk a sworn application, in writing, in duplicate, on a form to be furnished by the City Clerk, which application shall give the following information:
(1) The name and a description of the applicant;
(2) The address (legal and local) of the applicant;
(3) A brief description of the nature of the business and the goods to be sold, and, in the case of products of a farm or orchard, a statement as to whether or not such products are produced or grown by the applicant;
(4) If employed, the name and address of the employer, together with credentials establishing the exact relationship;
(5) The length of time for which the right to do business is desired;
(6) If a vehicle is to be used, a description of the same, together with the license number or other means of identification;
(7) A photograph of the applicant taken within 60 days immediately prior to the date of filing the application, which photograph
be two inches by two inches, showing the head and shoulders of the applicant in a clear and distinguishing manner;
(8) The fingerprints of the applicant and the names of at least two reliable
owners of the
who will certify as to the applicant’s good character and business
, or, in lieu of the names of references, at the discretion of the Chief of Police, any other available evidence as to the good character and business
of the applicant as will enable an investigator to properly evaluate such character and business
; and
(9) A statement as to whether the applicant has been convicted of any crime, misdemeanor or violation of any municipal
, the nature of the offense, name of any applicable jurisdiction, case number and the punishment or penalty assessed therefore.
(b) At the time of filing the application, a fee, which
be set and
be amended by resolution of City Council from time to time, shall be paid to the City Clerk to cover the cost of the investigation described in § 850.05.
