3-1-2:   APPLICATIONS FOR LICENSES AND PERMITS:
   A.   Applications for all licenses and permits required by this chapter shall be made in writing to the Mayor in the absence of provision to the contrary. Each application shall contain:
      1.   The name and the age of the applicant;
      2.   The permit or license desired;
      3.   The location to be used, if any;
      4.   Zoning district, if any;
      5.   A plan depicting the front facade of the building within which the business of the applicant shall be conducted;
      6.   A plan depicting the location of the area anticipated to be used to provide parking for customers and employees of the business anticipated to be conducted by the applicant;
      7.   If items are to be sold or services are to be rendered, a common description of the type of items to be sold or the type of services to be rendered by the applicant in the conduct of the business anticipated to be conducted by the applicant; and
      8.   The time the business is anticipated to be open to the public.
   B.   Each application shall also contain the number of the certificates of registration required under the Retailers’ Occupation Tax Act (35 ILCS 120/1 et seq.), Service Occupation Tax Act (35 ILCS 115/1 et seq.), and/or Use Tax Act (35 ILCS 105/1 et seq.), if applicable. Each application shall contain such additional information as may be needed for the proper guidance of the City officials in the issuing of the license or permit applied for.
