§ 71.37 APPLICATION.

   (A)   Anyone desiring a permit to operate a motorized golf cart pursuant to this subchapter shall submit an application on the form authorized by the City Council.

   (B)   The application must include the following:

      (1)   Date;

      (2)   Applicant name and address;

      (3)   Insurance company and policy number; and

      (4)   Signature of applicant.

   (C)   The application may also require additional information as determined by the City Council.
