The operating permit may not be transferred. The permit shall include the following system performance requirements:
(A) System operating requirements;
(B) Monitoring locations, procedures, and recording requirements;
(C) Maintenance requirements and schedules;
(D) Compliance limits and boundaries;
(E) Reporting requirements;
(F) City notification requirements for non-compliant conditions;
(G) Disclosure, location, and condition of acceptable soil treatment and dispersal system site; and
(H) Descriptions of acceptable and prohibited discharges.
