All meetings shall be tape-recorded, and the tapes of all open meetings shall be made available for public examination in the office of the City Clerk. The Recorder shall prepare written minutes of all open regular and special meetings which shall be approved by the Council and made available for public inspection. Written minutes shall include the names of all Councilors present, all motions, revolutions, resolutions, orders, ordinances, and measures proposed and their disposition, the results of all votes, with the vote of each Councilor by name unless the vote is unanimous, the substance of the discussion of any matter, and references to any documents discussed.
