3-4-3: APPLICATION FOR CERTIFICATE; FEE:
   A.   Information Required: Applications for a certificate of registration shall be made upon a form provided by the chief of police and filed with the chief. The applicant shall truthfully state in full the information requested on the application as follows:
      1.   The name and address of the present place of residence of the applicant and the length of residence at such address and the social security number of the applicant.
      2.   The address of the place of residence of the applicant during the past three (3) years if other than the present address.
      3.   The date of birth of the applicant and marital status and, if married, the name of the spouse.
      4.   A current photo of the applicant.
      5.   The name and address of the person, firm, corporation or association by whom the applicant is employed or represents, and the length of time of such employment or representation.
      6.   The name and address of the employer of the applicant during the past three (3) years if other than the present employer.
      7.   A description sufficient for identification of the subject matter of the soliciting in which the applicant will be engaged.
      8.   The period of time for which the certificate is applied.
      9.   The date, or approximate date, of the latest previous application for a certificate under this chapter, if any.
      10.   A statement concerning whether or not a certificate of registration issued to the applicant under this chapter has ever been revoked.
      11.   A statement concerning whether or not the applicant has ever been convicted of a violation of any of the provisions of this chapter, or an ordinance of any other Illinois municipality regulating soliciting.
      12.   A statement concerning whether or not the applicant has ever been convicted of the commission of a felony under the laws of the state of Illinois or any other state or commonwealth, or federal law.
      13.   Any other information that the chief of police may deem necessary to process the application.
   B.   Fee: A nonrefundable application fee shall accompany the application for Certificate of Registration as shown in the Schedule of Fees found in Section 1-6-1 of the Village Code. Nonprofit charitable organizations that are registered with the Illinois Secretary of State shall be exempt from application fees.
   C.   Oath Required: All statements made by the applicant upon the application or in connection therewith shall be under oath.
   D.   Records Kept: The chief of police shall cause to be kept in his office an accurate record of every application received and acted upon, together with all other information and data pertaining thereto, and a record of all certificates of registration issued under the provisions of this chapter, and of the denial thereof. Applications for certificates shall be numbered in consecutive order as filed, and every certificate issued and any renewal thereof shall be identified with the duplicate number of the application upon which it was issued.
