(A) In order to acquire the permit, the person or firm desiring to make any excavation upon a city street shall make an application for a permit with the City Clerk/Administrator.
(B) This application shall contain the following information:
(1) Exact location and size of cut or excavation;
(2) Purpose for cut or excavation; and
(3) Drawing or sketch of cut or excavation proposed.
