(A) The statement of financial interests shall be filed on a form prescribed by the Ethics Board, or by the administrative official designated by the Ethics Board.
(B) The Ethics Board, along with the City Clerk, will annually review the list of officials and employees required to file annual disclosure statements, to determine whether the lists are complete and accurate. Within 90 days after it has been formed, and by February 1 each year thereafter, the Ethics Board, along with the City Clerk, must create a list of the names and offices, or positions, of all officials, employees and others required to file annual disclosure statements pursuant to § 39.45, and shall notify all such persons of their obligation to file a financial annual disclosure statement.
(C) The Ethics Board or the designated administrative official shall deliver a copy of the form to each officer and employee required to file the statement, by first class mail, email or hand delivery, no later than January 31 of each year.
(D) The failure of the Ethics Board or of the designated administrative official to deliver a copy of the form to any officer or employee shall not relieve the officer or employee of their obligation to file the statement.
