§ 53.04 RECORDS TO BE MAINTAINED.
   (A)   The person in charge of any premises on which interments are made shall not inter or permit the interment or other disposition of a body unless it is accompanied by a burial, removal or transit permit.
   (B)   The town representative shall endorse upon the burial permit, over his or her signature, the date of interment and return the permit to the local registrar of his or her district within ten days from date of interment.
   (C)   The town shall keep a public record of all bodies disposed of on the premises under the town's charge which record states the name of each deceased person, place of death, date of burial or disposal and the name and address of the undertaker.
   (D)   An undertaker who inters a body in a burial site which has no person in charge shall so endorse, sign and file the permit and write across the face of the permit the words "no person in charge".
