(A) The Public Works Director may revoke a permit, in accordance with the procedures of this subchapter, upon finding that the permit holder has violated any provisions of this chapter which includes but is not limited to:
(1) Failure to factually report the wastewater constituents and characteristics of the user's discharge;
(2) Failure to report significant changes in operations, or wastewater constituents and characteristics that might impact the user's discharge;
(3) Refusal of reasonable access to the user's premises for the purpose of inspection or monitoring; or
(4) Violation of conditions of the permit.
(B) Any permit holder whose permit has been revoked shall immediately cease all discharge of any industrial wastewater to the POTW.
(C) In the event of a failure of the user to comply voluntarily with the notification of permit revocation, the Public Works Director shall take such steps as necessary to insure compliance.
(D) Before any further discharge of industrial wastewater may be made by the user, he or she must apply for and obtain a new permit for industrial wastewater discharge, pay all charges that would be required upon initial application, and pay all delinquent fees, charges and such other sums as the permit holder may owe to the town. Costs incurred by the town in revoking the permit and disconnecting the industrial sewer shall be paid by the user before issuance of a new permit.
