(a) Applicants for a horse-drawn carriage license under this chapter shall file with the Village Manager a written application on a form provided by the Village Manager, which application shall be signed by the applicant if an individual, by all partners if a partnership, and by the president if a corporation, and set forth the following information: name, date of birth, social security number, and location or place of business of the applicant.
(b) Applicants for a horse-drawn carriage license under this chapter shall furnish to the Village Manager on the application a description of each carriage as required by the Village Manager.
(c) All applications for a horse-drawn carriage license under this chapter shall be filed with the Village Manager at least three days prior to the desired date of issuance of such license.
