§ 113.04  APPLICANT’S REQUIRED INFORMATION.
   (A)   The applicant shall give his or her complete identification, his or her signature, the name of his or her employer, the nature of the product or services in which he or she is interested, the names of the manufacturers of such products, or of the organization he or she is representing, a complete copy of sales tax information, proof of general liability insurance, and the proposed method of operation in the town.
   (B)   The applicant shall state the description and the number of vehicles, if any, intended to be operated, in peddling, soliciting, or canvassing in the town.
