§ 153.251  APPLICATION.
   (A)   The proceedings for amendment of this chapter shall be initiated by:
      (1)   A petition from the owner or owners of the property specified on the application;
      (2)   A recommendation of the City Planning and Zoning Commission; or
      (3)   By action of the City Council.
   (B)   An application for an amendment shall be filed with the Zoning Administrator on the prescribed form.
   (C)   All applications to change the wording of this chapter shall include the following information:
      (1)   The stated reason for the requested change;
      (2)   A statement on the compatibility of the requested change with the Comprehensive Plan;
      (3)   The portion of the existing chapter which is to be amended;
      (4)   The proposed amended text and statements which outline any other effects that the amendment may have on other areas of this chapter; and
      (5)   Any additional information requested by the Planning and Zoning Commission.
   (D)   All applications to change the boundaries of any Zoning District shall include the following information:
      (1)   The names, addresses and signatures of the petitioner or petitioners;
      (2)   The names and addresses of all property owners within the area to be rezoned, their signatures and a description of the property owned by each;
      (3)   A legal description of the property or properties to be rezoned and a specific description of the area to be rezoned;
      (4)   The present and proposed district classifications of the area;
      (5)   The current and proposed use of the land;
      (6)   A copy of any deed restrictions or covenants pertaining to the property or properties to be rezoned;
      (7)   A statement explaining the reason for the requested change in zoning;
      (8)   A  statement on the compatibility of the requested change with the Comprehensive Plan;
      (9)   A map, plot plan or survey plot of the property or properties to be rezoned which illustrates the location, dimensions, zoning, existing uses and buildings located on adjacent properties within 350 feet; and
      (10)   Any additional information requested by the Planning and Zoning Commission.
