1373.04 SUBMITTAL REQUIREMENTS.
   The following shall also be submitted to the Gates Mills Planning and Zoning Commission:
   (a)   An affidavit from the applicant stating that there is a demonstrated need for the placement of the wireless communication tower in a residential district and that there is not a technically suitable space available on an existing tower to accommodate the need;
   (b)   A current certified survey showing existing lots and property boundaries, streets, rights of way, servitudes, buildings and building setbacks, surrounding streets, important physical features on the site, the total square footage of the site, the lot and square number and the name of the subdivision, if applicable;
   (c)   An affidavit which certifies that the proposed tower is located no less than 2,500 feet from another wireless communication tower within the Village;
   (d)   A basic site plan depicting the location and dimensions of all proposed structures, with setback dimensions, entrances and exits and the parking layout, including parking spaces;
   (e)   A landscape plan of the site delineating the location of existing trees to be preserved, the location and dimension of proposed planning areas, including the size, type and number of trees to be planted, curbs, fences, buffers and screening elevations of fences and materials used;
   (f)   The location of every tower, building or structure that could support the proposed antenna so as to allow it to serve its intended function. The applicant must demonstrate that a technically suitable location is not reasonably available on an existing tower, building or structure within the search area.
   (g)   The elevation of existing and proposed structures, showing the width, depth and height of the structures, the use, types of material and color schemes of the structures and statistical data on the tower and its related accessory structure;
   (h)   The zoning classification of the site and the zoning classification of all property within 500 feet of the proposed site;
   (i)   A utility inventory showing the location of all utilities impacting the site;
   (j)   An affidavit that the proposed tower meets all the requirements of the various State and Federal regulatory agencies involved and complies with standards set by the Federal Communications Commission (FCC);
   (k)   A report from a licensed professional engineer which describes the tower height and design, including a cross-section of the structure; demonstrates the tower's compliance with applicable structural standards; and describes the tower's capacity, including the number and type of search areas that can be accommodated;
   (l)   An environmental statement fully describing the effects that the proposed tower will have on the environment and surrounding area, including the radiological impact on human health and the impact on adjacent residential structures and streetscapes;
   (m)   A list of all property owners and addressees within 1,000 feet of the proposed wireless communication facility;
   (n)   A list of competitors, together with their addresses, so that the Village may notify the competitors of the application in an effort to encourage co-location; and
   (o)   Photographs showing structures on and uses of adjacent parcels.
