931.04 PERMIT APPLICATION; INFORMATION REQUIRED; REVIEW.
   An application for a permit under this chapter, in a form prescribed by the Service Director, shall be signed by the person, organization, partnership, installer or contractor who will perform the work of installing the water service connection. The application shall clearly identify and state the location of the connection and the fixtures to be connected; the size of such connection being applied for; an identifying number for the relevant lot to which such connection is being made; and a statement that prior to or concurrently with the filing of such application the applicant has also applied to the City of Cleveland Division of Water for permission to make such connection. The application shall be filed by the applicant with the Service Director who shall, within thirty days of such receipt examine such application, and, if it is in conformity with the requirements of this section, shall determine the number of service units attributable to the proposed connection as determined pursuant to Section 931.03, whether the relevant lot is within or outside the Village corporate limits, and determine the connection charge due under Section 931.02. Such resulting amount shall be due and payable to the Village by the applicant prior to the issuance of such permit to the applicant by the Service Director. The Service Director shall maintain such records as necessary in order to carry out the purposes of this chapter.
