Application for license as a gasfitter, appliance installer, gasfitting contractor, or appliance installation contractor shall be made upon blanks furnished by the Finance Officer and shall state the name of the person desiring such license and the desire to engage in gasfitting, appliance installation, gasfitting contractors, or appliance installation contractors. The proposed place of business and the name under which the business is to be carried on shall be stated.
