767.12 RECORDS REQUIREMENTS.
   (a)    The provider shall maintain an enrollment record on file for each child in his/her care by the first day of attendance containing the following information:
      (1)    The child's name, address, birth date, date of enrollment and the first day of attendance.
      (2)    The names, addresses and telephone numbers of each parent.
      (3)    The name, telephone number and address of the place of employment or location where the parent can be contacted each day the child is in care.
      (4)    The names, addresses, telephone numbers and relationships to each child of at least two (2) individuals who could be contacted in the event of an emergency if the parent cannot be reached.
   (b)    Medical, Dental and Emergency Records.
       (1)    The permit holder must maintain a record of the following information for each child in care:
         A.    A list of all allergies and any special precaution or treatment indicated for these allergies.
          B.    A list of any medications currently being administered to the child.
          C.    A list of any chronic physical problems and/or psychological problems and any history of hospitalization.
         D.    A description of any modified dietary restrictions.
         E.    An Emergency Transportation Authorization record, including:
            1.    The child's name, address and telephone number.
            2.    The parent's work or school address and telephone number.
            3.    Name, addresses and telephone numbers of the child's physician or clinic and dentist.
            4.    The names, addresses, telephone numbers and relationships to the child of at least two (2) persons who could be contacted by the provider in the event of an emergency if the parent cannot be contacted.
            5.    The Emergency Transportation Authorization Record shall be signed and dated by the parent, and shall either expressly grant consent to transport the child to a designated source or sources of emergency medical or dental treatment or expressly refuse to grant consent to transport the child to a source of emergency treatments and shall include specific instructions to the provider in the event of illness or injury requiring emergency medical or dental treatment.
             6.    The provider shall send the child's health record information with the child who is being transported to a source of emergency assistance.
            7.    The child's health record shall be confidential, except that it may be disclosed to any emergency contact or designated or emergency health care provider.
   (c)    Emergency telephone numbers for the emergency squad, Fire Department, hospital, poison control center, children's protective services agency and Police Department, or 911 shall be posted by a telephone that is accessible to the provider and any other persons in the home.
 
   (d)    The provider shall file an injury/illness report with the Garfield Heights Police Department when any of the following occur:
       (1)    A child is transported in accordance with this rule to a source of emergency assistance.
       (2)    A child receives a bump or blow to the head.
      (3)    An unusual or unexpected incident occurs which jeopardizes the safety of a child or staff, such as a child leaving the home unattended.
      (4)    A serious incident, injury or illness to a child. A serious incident, injury or illness includes any situation that requires a child to be removed from the home for medical treatment, professional consultation or transported for emergency treatment.
 
   (e)    A copy of the injury/illness report shall be given to the child's parent on the day of the incident/injury and kept on file in the provider's home.
 
   (f)    All records required to be kept pursuant to this Chapter shall be available for inspection by the Mayor or his designee.
