§ 111.41 INFORMATION REQUIRED FOR PERMIT.

   The prior written authorization required herein shall be in the form of a written permit, which shall include, but shall not be limited to, the following information:

   (A)   Date of permit;

   (B)   Name and address of permittee;

   (C)   Description of forest product:

      (1)   Species; and

      (2)   Product (saw logs, poles, posts, Christmas trees, ornamentals, fuel wood, and the like).

   (D)   Description of lands from which product will be taken (legal and geographic);

   (E)   Expiration date;

   (F)   Signature of administrator or land owner of federal lands and the respective addresses of same; and

   (G)   Signature of permittee.
