(A) Application for a license under this chapter shall be made at the office of the City Clerk. Blank applications for this purpose shall be issued by the City Clerk. The applications shall contain information as shall be required by the Clerk or duly authorized representative, and shall further contain an agreement by which the applicant consents to have the inspectors of the Departments of Health, Police, Fire and Buildings to enter, examine, and inspect any part of the premises or vehicles used in conducting the business or any product or food manufactured or sold, and to take samples for the purpose of analysis. Each application shall further contain an agreement by the applicant to conform to the rules and regulations of the Department of Health relative to the conduct of the business for which he is seeking a license. (B) (1) The application for a license shall contain at least the following information.
(a) Name and address of the applicant, age, date and place of birth.
(b) Prior convictions of applicant, if any.
(c) Place where machine or device is to be displayed or operated and the business conducted at that place.
(d) Description of machine to be covered by the license, mechanical features, name of manufacturer, serial number.
(2) No license shall be issued to any applicant unless he shall be over 21 years of age and a citizen of the United States.
